You were hired by the county health department in New York to head their nursing department. They are particularly concerned with differences between the county and the state on a number of health indicators. The health department data they are using is based on a county-wide health assessment that includes a number of data sources. The overall findings have the county just slightly below the state on a number of parameters, such as asthma and hypertension, but there is a much higher prevalence rate of diabetes. The director has asked you to examine these data more carefully to determine if any action is needed.

Next, you bring together a team of nurses in your department and conduct a review of the demographics of your county. You find that 35% of the population are Hispanic, 60% are non-Hispanic Whites, 3% are African American, and the remaining population is Asian or American Indian. When you examined the available health-related data based on racial groups, you found that the prevalence rate for type 2 diabetes in Hispanics over age 17 was double that of non-Hispanic whites (20.4% vs. 9.3%). At the national level, the prevalence rate is not so disparate (13.2 vs. 7.6). For those 50 years of age or older, 50% had been diagnosed with type 2 diabetes.
